The Single Assessment process requires collaboration from all agencies involved in the child’s life. Contributions may be direct or indirect. Schools, midwives and staff from 0-19 Health Child Service are invited to make direct written contributions, and update their contributions at review points. Contributors are sent a standardised template through secure email.
